Geostigma is a disease lingering throughout the Earth's lifestream, created when Jenova's cells mixed with it in the game. You don't have to "touch" the lifestream to get Geostigma, keep in mind the lifestream is what guides the flow of the worlds, it guides all life on the planet and is the origin and end of all living creatures. The children have it more frequently than adults because being more recently born, there are more of them around to catch the disease. It's a disease of the entire planet and the lifestream that flows throughout it, it's not like "Oh I touched that doorknob someone sneezed on and now I've got a cold." The children, and others who manage the disease have jenova's cells in their bodies, and geostigma is what happens when the body tries to fight off the foreign substances (Jenova's cells, obtained via the lifestream---their origin.)

why does cloud see those spots whenever his geostigma is touched? and who is the black haired guy at the end who waves to Cloud and leaves with Aeris?

The spot things are probably just like a seizure or something...unless someone else has a better explanation.

The black-haired guy is Zack, and boy is there a lot of backstory behind him...Don't read on if you ever plan to play the game, cause it spoils things. Of course, it's not like this thread is spoiler-free anyway.

He's the old boyfriend of Aeris. He was First Class in SOLDIER (Shinra's elite military). He fought Sephiroth when he first went crazy, got beat, then Cloud (a regular soldier at the time) killed Sephiroth (or so we thought). However, both Cloud and Zack, critically wounded, are caught by Shinra and are experimented on. Zack breaks free and takes Cloud with him, however Cloud is practically unconscious the whole time. Zack ends up bringing Cloud to Midgar before getting wounded and shot down my Shinra soldiers. Afterwards Cloud gains consciousness, but kinda goes crazy. He is very much under the influence of Jenova cells, and he forgets everything that happened, instead taking Zack's memory for his own. Cloud shows up in Midgar and Tifa hires him.

And thus the game begins, where they go on to take up days upon days of my time. The end.

Yeah, being that it's already been said, and that you shouldn't even watch the movie unless you've played the game I'm assuming it's safe to just say things like that. Zack's basically who Cloud thought he was, in a nutshell. Cloud was not born in Niblehem (or whatever the town is) with Tifa, Cloud was not first class of Soldier or any of that, Cloud's a rogue Sephiroth clone, and he went insane as hell and just kinda replaced himself with Zack, as has been said. Also Zack, being a kind of "big brother" to Cloud while they were in soldier, influenced Cloud in such ways as---Cloud uses Zack's sword, clothing style, everything pretty much. It's really cool in the game when you realize Cloud isn't this big macho town hero renegade guy, he's actually a really weird halfway superpowered Sephiroth clone who's been fooling himself for several years. It's awesome.
